Key Responsibilities
- Develop and execute tailored care plans that address the diverse needs of our members throughout the healthcare continuum.
- Conduct both telephonic and in-person clinical assessments to identify, evaluate, and manage the needs of our members.
- Coordinate care for high-risk individuals, including those with chronic illnesses, co-morbidities, or disabilities.
- Manage healthcare services effectively to promote efficient use of benefits and address gaps in care.
- Facilitate the implementation of care plans, including authorizations and referrals.
- Collaborate with Medical Directors, Physician Advisors, and interdisciplinary teams to optimize treatment strategies.
- Ensure compliance with all relevant regulations, policies, and procedures while troubleshooting provider and service-related challenges.
- Active and unrestricted Registered Nurse (RN) license; Compact/Multi-State license is mandatory.
- A Bachelor of Science in Nursing (BSN) is preferred.
- Relevant nursing experience, particularly in case management, care coordination, or patient advocacy.
- Prior exposure to medical/clinical cases involving pregnant and postpartum women.
- Familiarity with the holistic needs of patients, including physical, psychological, and socio-cultural aspects.
- Strong communication, analytical, and problem-solving capabilities.
- Ability to work independently as well as collaboratively in a fast-paced remote environment.
- An active and unrestricted RN license is essential.
- A BSN is preferred to enhance the qualification profile for this role.
- Experience in nursing with a specific focus on case management or care coordination is crucial.
- Maternity experience (Labor & Delivery, Postpartum, Mother/Baby) is advantageous.
