Key Responsibilities
- Conduct thorough health assessments and develop tailored strategies to manage and improve patients’ health.
- Perform 3 or more patient visits daily, utilizing both virtual methods and in-person interactions as necessary.
- Educate and counsel patients and their families regarding available resources and benefits.
- Maintain accurate and timely patient documentation, including medical histories, assessments, and care plans.
- Schedule and conduct follow-up appointments to monitor patient progress.
- Ensure compliance with quality care standards and best practices.
- Assist patients in accessing educational resources and support services.
- Engage actively in integrated care team meetings to foster collaborative patient care.
- Assess health status, execute care plans, and evaluate ongoing patient progress.
- Complete all required documentation promptly and other assigned duties.
- Active and unrestricted Registered Nurse (RN) license.
- Reliable transportation with a valid driver's license and car insurance.
- Proficiency in operating necessary medical instruments and equipment.
- Familiarity with computer-based data management systems, medical records, and point-of-interview technology.
- Exceptional verbal and written communication skills.
- Strong understanding of federal and state regulations, including HIPAA and OSHA.
- Minimum of 2 years of direct patient care experience.
- An active and unrestricted Registered Nurse (RN) license is mandatory.
- A minimum of 2 years of experience in direct patient care is required.
