Job Summary

As a Community Behavioral Health Support Advocate, you will play a crucial role in helping individuals live their lives to the fullest. By promoting recovery and resiliency in the least restrictive environment, you will coordinate and collaborate with various partners, including consumers, their families/caregivers, residential providers, medical and behavioral health professionals, and Home and Community Services (HCS) case managers.

Key Responsibilities

Manage Requests: Handle HCS requests for Supportive Supervision and Behavioral Health Personal Care benefits within designated timeframes.

Clinical Review: Use advanced clinical skills to evaluate Care Assessments and related information to make informed decisions.

Assessment and Referral: Assess members receiving Supportive Supervision benefits, identify additional treatment services to promote recovery and resiliency, and make proactive referrals as needed.

Connection Support: Assist members and their families/caregivers in connecting with appropriate psychiatric, medical, and psychosocial services.

Barrier Removal: Identify and address barriers to accessing, delivering, participating in, and succeeding in services.

Follow-Up and Monitoring: Provide supportive follow-up, monitoring, and education both in-person and telephonically to members, guardians, caregivers, and residential providers.

Collaborative Partnership: Work with external providers and programs to address members' needs, care gaps, and recovery goals.

Case Facilitation: Facilitate case conferences with Behavioral Health providers, HCS case managers, medical providers, inpatient facilities, and residential providers.

Performance Review: Participate in meetings to review performance, identify areas for improvement, and guide enhancement efforts.

Compliance: Adhere to all policies, procedures, and documentation standards, ensuring proper tracking and database management.

Consumer Engagement: Motivate and engage consumers through a recovery, health, and wellness-oriented approach.

Service Coordination: Collaborate with providers to assess mental health concerns, address barriers, and coordinate appropriate services.

Required Qualifications

Professional Standards: Mental Health Professional per WAC 246-341-0515 (4).

Experience: At least 2 years of experience serving the Medicaid population, preferably in a community mental health setting, and 2 years of experience with Adults with Serious and Persistent Mental Illness.

Technical Skills: Proficiency in computer/typing for data entry in electronic clinical records, email, internet research, online calendars, and other software applications.

Internet Access: Reliable high-speed internet from home.

Communication Skills: Exceptional written and verbal communication skills.

Preferred Qualifications

Education and Licensing: Licensed Master's degree in Psychology, Social Work, Counseling, Marriage or Family Counseling, or a Licensed Ph.D., or an RN with 2 years of experience in behavioral health. Licenses must be active, independent, and unrestricted in the state of Washington.

Specialized Experience: Experience with dual diagnosis of mental health and substance abuse, community mental health settings, and managed care environments.

Language Skills: Bilingual skills (fluency in Spanish/English).
