Key Responsibilities
- Serve as an integral member of a collaborative care team, working alongside a Registered Nurse and Social Worker.
- Deliver personalized medical care to patients diagnosed with chronic kidney disease (CKD) and end-stage renal disease (ESRD), facilitating their journey towards improved health from the comfort of their homes.
- Conduct comprehensive assessments, including annual wellness exams, both in-person and via telehealth.
- Educate patients and their families on available benefits and resources.
- Document patient encounters meticulously, encompassing all aspects of their care.
- Coordinate care with primary care providers, specialists, and ancillary services to promote seamless healthcare delivery.
- Uphold the highest standards of medical care and evidence-based practice.
- Prescribe medications and order tests in collaboration with the patient’s physician.
- Contribute to enhancing patient outcomes through education and preventive measures.
- Participate actively in integrated care team meetings, fostering collaboration and communication.
- Bilingual proficiency in English and Spanish.
- Valid and unrestricted RN and Nurse Practitioner or Physician Assistant license.
- Board certification (NP: ANCC/AANP; PA: NCCPA).
- Current and valid DEA certificate.
- Strong independence and autonomy in managing patient care.
- Reliable transportation, valid driver's license, and car insurance.
- Proficient in using medical instruments and technology.
- Exceptional verbal and written communication skills.
- Knowledgeable in federal and state regulations (HIPAA and OSHA).
- Minimum of two years of direct patient care experience.
- An advanced degree in Nursing or a related field.
- Experience in managed care settings and conducting comprehensive wellness visits.
