We are in search of a committed Home-Based Clinical Therapist to enhance our team. In this role, you will deliver vital therapeutic services and support to families within the comfort of their homes, enabling them to effect meaningful change.

Key Responsibilities

• Conduct assessments of family needs through interviews and validated assessment tools, while considering social histories and collaborating with community resources.
• Formulate and execute personalized treatment plans with specific, measurable goals.
• Provide a spectrum of therapeutic interventions, including individual, family, and group counseling sessions, directly in the family’s home environment.
• Deliver case management services as required, connecting families to essential resources within the community and the county’s support network.
• Partner with other agencies and stakeholders involved in the family’s care to ensure a cohesive, comprehensive approach.
• Engage in multidisciplinary case staffing to facilitate integrated and holistic care.
• Maintain accurate and timely documentation of all services rendered.
• Prepare reports as requested to support effective communication among team members.
• Offer crisis intervention as necessary, providing support and direction during challenging times.
• Adhere to all safety protocols and procedures to ensure a secure and nurturing environment for families.
• Perform additional related duties as assigned.
  
  

Required Skills

• A Master’s degree in counseling, social work, or psychology.
• Proven experience in providing individual, family, and group therapy to a diverse clientele.
• Experience in working with youth and families, especially those involved in the juvenile justice system, is advantageous.
• Reliable transportation, a valid driver’s license, and auto insurance for conducting home visits.
• Licensure/registration in your designated area.
• Proficient computer skills and familiarity with electronic health records.
• Strong written and verbal communication capabilities.

• A Master’s degree in counseling, social work, or psychology is mandatory.
