Key Responsibilities

• Conduct initial nursing assessments for new patients.
• Develop, coordinate, and update comprehensive care plans in collaboration with the healthcare team to ensure seamless continuity of care.
• Communicate patient progress regularly with the clinical manager, physician, and care team.
• Maintain accurate and timely records of patient care, documenting observations, interventions, and outcomes.
• Effectively manage patient pain and symptoms, ensuring the highest quality of life and comfort.
• Educate families on patient conditions and care needs, empowering their active participation in the care process.
• Refer patients to other hospice care disciplines as necessary.
• Provide on-call support and services to patients and families as assigned, responding to their needs with empathy and efficiency.

Required Skills

• Active RN license specific to your state of employment.
• Minimum of one year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Strong communication and interpersonal skills.

Compensation And Benefits

• Competitive salary ranging from \(80,000 to \)85,000 per year, commensurate with experience and location.
• Generous sign-on bonus of $10,000.
