Key Responsibilities
- Conduct initial nursing assessments for new patients.
- Develop, coordinate, and update comprehensive care plans in collaboration with the healthcare team to ensure seamless continuity of care.
- Communicate patient progress regularly with the clinical manager, physician, and care team.
- Maintain accurate and timely records of patient care, documenting observations, interventions, and outcomes.
- Effectively manage patient pain and symptoms, ensuring the highest quality of life and comfort.
- Educate families on patient conditions and care needs, empowering their active participation in the care process.
- Refer patients to other hospice care disciplines as necessary.
- Provide on-call support and services to patients and families as assigned, responding to their needs with empathy and efficiency.
- Perform additional duties as required.
- Active RN license specific to your state of employment.
- Minimum of one year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Strong communication and interpersonal skills.
- Profound empathy and compassion for patients and families.
- A commitment to providing outstanding hospice care.
- Associate’s degree in Nursing or higher.
- One or more years of relevant nursing experience.

Compensation And Benefits
- Competitive salary ranging from \(80,000 to \)85,000 per year, commensurate with experience and location.
- Generous sign-on bonus of $10,000.
