Key Responsibilities
- Conduct initial nursing assessments for new patient admissions.
- Develop, coordinate, and revise comprehensive care plans, ensuring continuity of care through collaboration with the appropriate staff.
- Maintain regular communication regarding patient progress with the clinical manager, physician, and care team.
- Accurately create and maintain individualized patient care progress records in a timely manner.
- Document changes and outcomes in pain and symptom management as necessary.
- Educate families on the patient’s condition, needs, and ongoing care.
- Refer patients to other hospice care disciplines when needed or document reasons for not doing so.
- Fulfill on-call responsibilities and provide services to patients and families as assigned.
- Perform additional duties as required.
- Current RN license in PA.
- A minimum of one year of nursing experience.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- An accredited nursing degree, either an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
