Key Responsibilities
- Deliver personalized, compassionate care to patients in a primarily virtual setting.
- Conduct in-home assessments, including comprehensive annual wellness exams and focused screenings as necessary within a designated geographic area.
- Complete a minimum of three patient visits daily.
- Provide counseling and education to patients and their families regarding available benefits and programs that promote healthier living.
- Accurately document patient information, including chief complaints, diagnoses, medical history, examinations, medications, allergies, assessments, and care plans.
- Ensure timely and meticulous completion of all documentation.
- Uphold high standards of quality care.
- Assist patients in accessing educational resources pertaining to their health.
- Engage actively in integrated care team meetings.
- Continuously assess patients’ health status and develop tailored care plans.
- Monitor and evaluate patient progress.
- Execute other duties as assigned.
- Active and unrestricted Registered Nurse license.
- Reliable access to transportation, including a valid driver’s license and car insurance.
- Proficient in using necessary medical instruments and equipment.
- Familiarity with computer-based data management systems, medical records, and point-of-interview technologies.
- Exceptional verbal and written communication skills to effectively interact with patients, families, healthcare providers, and community representatives.
- Comprehensive understanding of federal and state regulations, including HIPAA and OSHA compliance.
- A minimum of two years of direct patient care experience.
- Must possess an active and unrestricted Registered Nurse license.
- A minimum of two years of experience in direct patient care is required.
