Key Responsibilities
- Conduct thorough assessments of family needs through interviews and established assessment tools, taking into account social history and available community resources.
- Create and execute tailored treatment plans with clear, measurable goals and objectives.
- Deliver a variety of therapeutic interventions, including individual, family, and group counseling sessions in clients’ homes.
- Offer case management support, connecting families to essential community resources and services.
- Collaborate with other agencies and stakeholders involved in the family’s care to ensure a cohesive and comprehensive approach.
- Participate in multidisciplinary case reviews to foster integrated and holistic care.

- Reliable transportation, a valid driver’s license, and appropriate auto insurance for home visits.
- Current licensure/registration within your designated area.
- Proficient computer skills and familiarity with electronic health records.
- Strong written and verbal communication abilities.
- Master’s degree in counseling, social work, or psychology is required.
- Demonstrated experience in providing therapy to various populations.
- Experience with youth and families is preferred but not mandatory.
