Key Responsibilities
- Engage with members face-to-face about preventive health services, maternity care, care transitions, and chronic disease initiatives.
- Educate individuals on the significance of preventive health initiatives, assess their needs, and facilitate appropriate referrals, including behavioral health support when necessary.
- Coordinate access to vital preventive health services through outreach to members, providers, and community organizations.
- Foster strong relationships with members and local community centers to create empowering and supportive connections.
- Assist members in completing health risk assessments, identify and address healthcare barriers, schedule appointments, and respond to inquiries.
- Collaborate with human service providers to identify and assist members requiring support.
- Provide accurate information regarding available services and benefits.
- Contribute to the creation of educational resources for both members and providers.
- Conduct training sessions for providers and community groups focused on preventive health initiatives.
- Develop effective and measurable programs, particularly targeting at-risk populations.
- Represent the organization at community events to promote health awareness.
- High School Diploma or GED.
- Act 33⁄34 and 73 clearances (must be obtained prior to starting if offered a position).
- Experience in a health-related customer service or marketing environment is preferred.
- Medical assistant experience is required.
- Call center experience is required.
- Strong capacity for problem-solving, with a proactive and creative mindset.
- Excellent written and oral communication skills.
- A deep appreciation for cultural diversity and sensitivity towards varying populations, including those with limited English proficiency.
- Demonstrated patience and empathy in interactions with members and customers.
- High School Diploma or GED.
