Key Responsibilities
- Deliver skilled nursing care to patients of all ages in their homes, under the supervision of a Registered Nurse.
- Collaborate with an interdisciplinary team, patients, families, and referring agencies to coordinate care effectively.
- Assist the Registered Nurse in assessing patients’ physical, psychosocial, and environmental needs through various documentation and evaluations.
- Participate in the planning and implementation of individualized care plans alongside the Registered Nurse.
- Uphold quality standards by employing infection control measures to safeguard both staff and patients.
- Communicate proficiently with patients, families, team members, and healthcare professionals through clinical documentation and discussions.
- Monitor assigned cases to ensure adherence to care requirements and quality standards.

- Strong nursing skills consistent with accepted standards of practice.
