Key Responsibilities
- Conduct thorough health assessments to formulate strategies for patient health improvement and management.
- Perform an average of 3 patient visits daily, utilizing both virtual and in-person methods as required.
- Counsel patients and their families regarding available benefits and support programs.
- Maintain precise and timely patient records, including complaints, diagnoses, medical history, and care plans.
- Organize and manage follow-up appointments to ensure continuity of care.
- Uphold quality care standards and assist patients in gaining access to educational resources.
- Actively engage in integrated care team meetings to enhance collaborative efforts.
- Monitor patient health status, implement care plans, and follow up to assess progress.
- Complete all necessary documentation promptly and efficiently.
- Undertake additional responsibilities as assigned.
- An active and unrestricted Registered Nurse (RN) license.
- Reliable transportation, along with a valid driver’s license and car insurance.
- Proficient knowledge of medical instruments, data management systems, and medical records.
- Strong verbal and written communication capabilities.
- Familiarity with federal and state regulations, including HIPAA and OSHA.
- A minimum of 2 years of direct patient care experience.
- A nursing degree from an accredited program.
- Active and unrestricted Registered Nurse (RN) license.
- 2 years of direct experience in patient care settings.
