Key Responsibilities
- Conduct comprehensive assessments and annual wellness exams for patients, both in-person and virtually.
- Educate patients and their families about available healthcare benefits and support programs.
- Maintain detailed and accurate medical records including assessments and treatment plans.
- Collaborate with a multidisciplinary team, including primary care providers and specialists, to ensure holistic patient care.
- Complete all necessary documentation promptly and accurately.
- Adhere to quality care standards in every aspect of patient interaction.
- Deliver evidence-based care aimed at minimizing hospitalizations and enhancing patient satisfaction.
- Prescribe medications and coordinate diagnostic testing in collaboration with patients’ healthcare teams.
- Promote patient education by facilitating access to relevant resources.
- Contribute actively to care team meetings and discussions.
- Active and unrestricted RN and NP or PA license.
- Board certification relevant to your licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Proficient in utilizing medical instruments and technology for patient care.
- Solid experience with data management systems and medical records.
- Excellent communication skills, both verbal and written.
- Knowledge of HIPAA and OSHA regulations.
- Bilingual skills (English/Spanish) are highly desirable.
- Minimum of two years of direct patient care experience.
- Experience with managed care environments and conducting wellness visits, either virtually or in-home.
