Positioned in Jefferson Hills and Bethel Park, PA, our full-time RN Case Manager role allows you to deliver empathetic, individualized care to patients within the comfort of their homes.

Key Responsibilities

• Conduct comprehensive patient evaluations and collaborate with the interdisciplinary care team to develop and execute tailored care plans.
• Facilitate referrals to other healthcare professionals as necessary or document the rationale for opting out.
• Preserve accurate and timely patient care documentation in alignment with organizational policies.
• Document changes and outcomes related to pain and symptom management.
• Provide education to patients, families, and caregivers regarding effective care techniques.
• Maintain regular communication with the clinical manager and care team.
• Fulfill on-call responsibilities and deliver assigned on-call services during weekdays and weekends as scheduled.
• Undertake additional duties as required.
  
  

Required Skills

• A minimum of one year of nursing experience.
• Valid and current RN license in Pennsylvania.
• Current CPR certification.
• A valid driver's license, reliable transportation, and liability insurance.
