Key Responsibilities
- Conduct thorough nursing assessments for new admissions and collaborate with the care team to develop and execute personalized care plans.
- Maintain open communication regarding patient progress with the clinical manager, physician, and care team to ensure a coordinated approach to care.
- Accurately document patient care activities, progress, and changes in a timely manner, adhering to established policies and procedures.
- Monitor and manage pain and symptom relief, adapting care plans as necessary to enhance patient comfort.
- Educate and empower families with the knowledge and support needed as they navigate the hospice journey.
- Facilitate connections between patients or families and other hospice care specialists to ensure comprehensive support.
- Provide responsive on-call support to patients and families, addressing urgent needs with compassion and guidance.
- Current RN license in PA.
- Minimum of one year of nursing experience.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- A nursing degree from an accredited program.
