Key Responsibilities
- Utilize risk stratification tools to identify high-risk patients.
- Conduct thorough assessments and consultations, developing treatment and discharge recommendations, including remote patient monitoring.
- Facilitate complex care planning through telehealth, phone consultations, and in-person meetings.
- Coordinate all facets of patients’ transitions of care, which include follow-up appointments, home health services, community resources, and transportation needs.
- Enhance communication across various care settings (acute care, ambulatory, skilled nursing, palliative care, and hospice).
- Empower patients through educational initiatives focusing on disease management, medication adherence, access to care, and self-care strategies.
- Assess patient needs for referral to supplementary programs and services (e.g., social workers, community agencies).
- Collaborate regularly with inpatient teams, primary care physicians, and other stakeholders to ensure the relevance and feasibility of transition plans.
- Build rapport with patients in hospital settings to facilitate smooth transitions to outpatient care.
- Maintain solid relationships with patients, families, community resources, and payers.
- Implement comprehensive care plans that prioritize patient goals and needs.
- Lead initiatives for program improvement and team education.
- Perform additional duties as assigned.
- Proficient critical thinking and communication skills, both written and verbal.
- In-depth knowledge of discharge planning principles and guidelines.
- Strong collaborative skills to engage effectively with internal and external partners.
- Adaptability in dynamic and crisis environments.
- Excellent decision-making capabilities.
- Outstanding time management and prioritization skills.
- Proficiency in computer applications (e.g., word processing, data management).
- Associate Degree in Nursing (mandatory).
- Minimum of three (3) years of nursing experience (required).
- At least one (1) year of case management experience (required).
- Current Maryland RN license (required).
- BLS certification (required).
- Preferred: Bachelor of Science in Nursing.
