This role offers the opportunity to integrate care for both mental and physical health needs, ensuring holistic support for our clients.
Key Responsibilities
- Conduct initial and ongoing diagnostic assessments, addressing both mental and physical health symptoms.
- Collaborate with primary and specialty care providers to develop tailored treatment plans.
- Facilitate individual and group therapy sessions in various settings, including remote via telehealth.
- Provide psycho-education utilizing the Integrated-Illness Management and Recovery (I-IMR) model.
- Engage with families and community support systems to reinforce coping strategies.
- Participate in crisis intervention when necessary and maintain adherence to the Integrated Care Model.
- Uphold client confidentiality and comply with agency policies and standards.
- Master's degree in a mental health discipline (social work, clinical mental health counseling, psychology, etc.).
- Licensure as a Clinical Mental Health Counselor (LCMHC) or Licensed Independent Clinical Social Worker (LICSW) is preferred.
- Familiarity with chronic co-morbid physical health conditions and experience with medically complex individuals.
- Strong clinical judgment and effective communication abilities.
- Proficient in Microsoft Office and electronic health record systems.
- Ability to manage multiple tasks while maintaining attention to detail.
- A demonstrated commitment to ongoing professional development and learning new clinical competencies.
- Experience working collaboratively within a team as well as independently.
- Effective organizational skills and the capability to manage documentation efficiently.
