Role Description

This is a full-time on-site role for a Primary Care Physician in Fresno, CA. The Primary Care Physician will be responsible for providing primary care to seniors and specializing in family medicine and general medicine.

Qualifications

Primary Care, Family Medicine, and Medicine skills
Experience as a Primary Care Physician
Urgent Care expertise
Ability to diagnose and treat a wide range of illnesses and injuries
Excellent communication and interpersonal skills
Board certification in Family Medicine or Internal Medicine
Strong analytical and problem-solving skills
Current licensure as a Physician in California

Responsibilities

Provides assessment, planning, primary care, and continuous medical coverage to participants

Collaborates with community-based physicians and IDT in discharge planning from acute or long-term settings

Collaborates on care planning decisions and coordinating 24-hour care delivery

Helps maintain care of participants in the nursing home, making regular visits

Takes part in rotating night and weekend call
