Job Summary:
We are seeking a skilled Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide high-quality care in our long-term care setting. The ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering compassionate patient care.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Assess patients' needs, develop individualized care plans, and implement interventions to achieve desired outcomes
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Document patient information accurately and thoroughly in electronic medical records
- Maintain accurate medication administration records, incident reports, and other relevant documentation
Requirements:
- Degree: Associate's degree or diploma in nursing from an accredited program
- Licensure: Current LPN/LVN licensure in the state of Minnesota
- Certifications: BLS certification; other certifications may be required based on unit/department
- Experience: Minimum 1 year of experience in long-term care setting preferred
