About the Role:
Are you a dedicated Registered Nurse looking for a unique opportunity to deliver personalized care and guide patients through their post-acute recovery journey? This role focuses on empowering patients and their families while ensuring they have the resources and support needed for a successful recovery.
Key Responsibilities:
- Serve as a key liaison between patients, families, and the healthcare team to deliver exceptional care and foster communication.
- Develop and manage individualized care plans, ensuring they align with clinical standards and patient goals.
- Provide education and support to patients and their families about the recovery process, medication management, and potential complications.
- Collaborate with physicians and other care providers to monitor and adjust care plans based on patient progress.
- Utilize data and metrics to assess outcomes, identify trends, and implement quality improvements.
Schedule: 4(10) Hour Shifts Per Week, Day/Evenings available, Shift Differentials for Evenings!

Qualifications:
- ASN Required
- New York RN License Required
- 2 years acute case management experience required (Med/Surg, ICU, ED)
