Job Details
- Location: Jackson, MS
- Estimated Weekly Salary: $1900 to $2040
- Contract Duration: 13 weeks
- Shift: 8-hour days, Monday to Friday, 8 AM - 4:30 PM
- Shifts per Week: 5
- 2 years of experience in Acute Case Management
- Experience in a Level 1 Trauma or Large Academic setting
- Licensure: Mississippi LMSW or LCSW
- Proficiency with Epic EMR systems
- Cover inpatient units throughout the hospital (excluding ED)
- Manage medical and some social discharges
- Handle a caseload of 15-20 patients
- Collaborate with the Case Management team
