- Location: Fort Pierce, FL
- Duration: 13 weeks
- Start Date: February 10, 2025
- Shift: 8-hour days, 5 shifts per week
- Estimated Weekly Salary: $1690 - $1820
- Employment Type: Travel
- On-Site: Yes
- Minimum of 1 year of experience in acute care adult hospital case management
- Experience with MIDAS/Meditech, PASRR, and Navihealth
- MSW or LCSW preferred
- Knowledge of hospital discharge planning and execution
- Conduct discharge planning and manage complex cases
- Work collaboratively with family and patients to ensure optimal care
- Perform MDR, referrals, and participate in interdisciplinary team meetings
- Float to various units as needed, except trauma
