Nurse Practitioner-Primary Care Clinic

Monday - Friday, 8 AM - 5 PM
Morning huddles with the care team

Role Overview:
Join our primary care outpatient clinic serving Medicare patients aged 65 and older. Our focus is on value-based care, emphasizing quality over quantity. You'll spend more time with patients, helping to keep them healthy and out of the hospital.

Key Responsibilities:

• Provide exceptional primary care and conduct routine and acute visits.
• Administer annual wellness visits and health risk assessments.
• Coordinate care with specialists and ensure proper documentation.
• Lead population health initiatives and participate in phone triage.
• Educate patients on health conditions and care plans.
• Conduct home visits as needed.

Qualifications:

• Master’s in Nursing or NP certification.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care NP.
• Active state Nurse Practitioner license and DEA license.

Preferred Qualifications:

• Experience in primary care (internal medicine, geriatrics, family medicine).
• Familiarity with HCC documentation and ICD-10 coding.
• Passion for teamwork and innovative population health models.
• Bilingual proficiency is a plus.
