Nurse Practitioner - Primary Care

Join a dedicated, multi-state healthcare organization as a Nurse Practitioner in a primary care outpatient clinic located in Tucson, AZ. This permanent position focuses on providing effective and equitable value-based care to adults on Medicare, primarily those aged 65 and older. Our model emphasizes quality of care over quantity, allowing you to spend more time with patients and keep them healthy and out of the hospital.

Job Details

• Employment Type: Permanent
• Hours Per Week: 40
• Schedule: Monday - Friday, 8 AM - 5 PM
• Salary: $105,000 - $135,000/yr (Depending on Experience)

Job Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• At least 1 year of experience in primary care: internal medicine, geriatrics, or family medicine preferred
• Bilingual proficiency in applicable areas preferred

Responsibilities

• Provide exceptional primary care in a clinic setting
• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments
• Coordinate care with other healthcare providers, specialists, and facilities
• Lead population health initiatives in coordination with the Care Team
• Assist with phone triage and patient outreach
• Educate patients on health conditions, care plans, and treatments
• Participate in promotional activities and conduct home visits as needed
