Job Details

- Salary: $120,000 - $150,000 annually

Job Requirements
- Valid unencumbered Nurse Practitioner License for the state of Idaho
- Previous in-home risk assessment experience preferred
- 3 years of patient care experience preferred (primary care, adult, or geriatric settings)
- Access to reliable transportation
- Strong proficiency in EMR systems
- Ability to work independently
- Bilingual skills are a plus
Responsibilities
- Conduct annual in-home wellness visits and risk adjustment health assessments for populations with chronic conditions
- Provide personalized quality care to elderly patients in their homes
- Educate patients on health topics such as pain management and medication
- Build trust with members through excellent communication and empathy
- Assist in closing quality care gaps, including screenings and labs
- Collaborate with healthcare teams to enhance care delivery
- Travel within your assigned state and within a 55-mile radius
- Perform other duties as assigned
