- Salary: $120,000 - $150,000 annually
- Hours Per Week: 40
- Shift Duration: 8 Hour Days

Job Requirements
- Valid and unencumbered Nurse Practitioner License for the state of Idaho
- Minimum of 1 year of experience in patient care (primary care/adult/geriatric, EMR preferred)
- Previous in-home risk assessment experience preferred
- Access to reliable transportation
- Strong ability to work within an EMR system
- Ability to work independently
- Bilingual skills are a plus
Responsibilities
- Perform annual in-home wellness visits and risk adjustment health assessments
- Provide individualized quality care to the elderly population in their homes
- Deliver patient health education on topics such as pain management and medication
- Build relationships of trust with members through exceptional communication and empathy
- Assist in closing quality care gaps (i.e., screenings and labs)
- Collaborate enthusiastically to enhance care delivery
- Travel within the assigned state as required
- Other duties as assigned
