- Duration: 26 weeks
- Hours Per Week: 40
- Shift: 8 Hour Days, Monday to Friday, 8am - 5pm with one hour lunch
- On Call: Rotation via phone once every 7 weeks
- Estimated Weekly Salary: $5090 - $5470
Job Requirements
- Licensed Physician in the state of Pennsylvania
- Board certified in Internal Medicine or Family Practice
- BLS certification required
- Mission-driven to serve the underserved
- Experience and comfort utilizing EMR
- Ability to work as part of a multidisciplinary team
- At least 2 years of experience in Primary Care or Internal Medicine
- Preferred: Experience in population health management and working with psycho-socially and medically complex patient population
Responsibilities
- Manage and improve health for patients with multiple complex and chronic medical conditions
- Lead and collaborate with a team including social work, case management, and other specialists
- Develop strong transitional care plans and track patients across the continuum of care
- Communicate with alternate levels of care facilities to facilitate appropriate discharges
- Treat acute exacerbations of chronic conditions within the care center
- Accept urgent appointments to ensure acute episodes are resolved
- Educate members on chronic disease progression and advanced directives
- Practice evidence-based medicine in accordance with the PCMH model of care
