- Pay Rate: $45 per hour
- Location: South San Francisco, CA
- Duration: 13 weeks
- Start Date: February 10, 2025
- Shift: Day shift, 8-hour shifts, 5 days per week
- Potential Shift Times: 3pm-11:30pm and if Overnight – 11:00pm-7:30am
- Employment Type: Travel Nursing
- Minimum of 1 year of experience in Case Management
- Active Registered Nurse (RN) license
- Ability to work on-site in South San Francisco
- Strong communication and organizational skills
- Two professional references
- Coordinate patient care and manage cases effectively
- Develop and implement patient care plans
- Collaborate with healthcare providers to ensure optimal patient outcomes
- Conduct patient assessments and evaluations
- Maintain accurate and timely documentation
