We are seeking a dedicated and experienced Registered Nurse (RN) specializing in Case Management for a travel position in Indianapolis, IN. Job Details
- Location: Indianapolis, IN
- Employment Type: Travel
- Duration: 13 weeks
- Start Date: January 27, 2025
- Shift: Monday - Friday, 8:00 AM - 4:30 PM
- Hours per Week: 40 hours
- Estimated Weekly Salary: $1810 - $1940
- Must have a valid Indiana Registered Nurse (RN) license
- Basic Life Support (BLS) certification through the American Heart Association
- 2 years of recent hospital Case Management experience
- Experience in coordinating clinical and psycho-social activities
- Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians
- Develop and execute effective discharge planning
- Monitor patient stays for effective care coordination and efficient care facilitation
- Conduct medical necessity reviews for appropriate utilization of services
- Document all patient review records according to regulatory and legal requirements
- Conduct comprehensive biopsychosocial assessments of patients and families
- Serve as a patient advocate and enhance collaborative relationships
- Participate in interdisciplinary patient care rounds and conferences
- Initiate patient and family education and discharge planning at admission
