- Location: Washington, DC
- Duration: 13 weeks
- Employment Type: Travel
- Hours Per Week: 40
- Shift: 8-hour days, Monday to Friday, with a weekend rotation every 4th weekend (Saturday and Sunday)
- Start Date: February 10, 2025
- Estimated Weekly Salary: $1,865 - $2,072
- DC Social Worker License
- CPR Certification
- Covid Vaccination
- Experience with EMR-Cerner and Allscripts CM Software is preferred
- At least 1 year of experience in a similar healthcare role
- Conduct discharge planning and psychosocial assessments
- Collect and analyze patient information related to physical, psychological, socio-cultural, economic, spiritual, and lifestyle behaviors
- Provide relationship-based care that promotes continuous, consistent, and efficient delivery of care
- Identify and address complex problems of specific patient populations
- Incorporate evidence-based practice and research into patient care delivery
- Serve as a patient advocate and communicate clinical concerns to the interdisciplinary team
- Develop and individualize patient care plans
- Maintain a safe and therapeutic environment
- Participate in multi-disciplinary quality and service improvement teams
