GENERAL SUMMARY
Responsible for providing wound care to Wound Clinic patients in the hospital based outpatient wound clinic.

PRINCIPLE DUTIES AND RESPONSIBILITIES
1. Demonstrates effective communication skills with physicians, department and hospital staff, patients, caregivers and referral sources.
2. Provides comprehensive wound assessment and evaluation and works closely with the Wound Care Medical Director to plan, implement and coordinate the patient's needs.
3. Demonstrates competence in patient/wound assessment and dressing protocols /procedures and products.
4. Documents assessment and patient data following all existing structure standards and guidelines for forms
5. Evaluates effectiveness of care given by self and by all health care team members. Also, evaluates contribution of systems, environment, and instrumentation in processing patients toward desired outcomes.
6. Demonstrates competency in select psychomotor skills adhering to predefined written procedures.
7. Identifies patient learning needs and readiness for learning. Implements patient/significant other instruction to meet self-care needs as defined in standards of care and patient teaching protocols.
8. Demonstrates awareness and sensitivity to patient rights, as identified within the institution.
9. Functions with an awareness of and adherence to safety requirements as
identified in existing standards
10. Performs efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
11. Demonstrates applied knowledge base in areas of structure standards, patient care resources/references, protocols, pertinent drugs, standards of care, and significant lab values.
12. Demonstrates an awareness of self-responsibility and accountability for own professional practice within the Illinois Practice Act.
13. Participates actively in staff development activities by sharing knowledge with others and using appropriate teaching/learning strategies to assist peers.
14. Demonstrates self directed learning and participation in continuing education to meet own professional development.
15. Seeks validation of knowledge base, skill level, and decision making as necessary and assertively requests guidance in areas of question.
16. Demonstrates awareness of legal issues in all aspects of professional practice and patient care and strives to manage situations in a reduced-risk manner.

22. Formulates and uses effective working relationships with all health care team members, patients, and families.
23. Demonstrates appropriate use of lines of authority in clinical and operational matters when seeking direction, validating self decision making, and resolving problematic issues.

Qualifications
PHYSICAL REQUIREMENTS
1. Requires the ability to stand 40% of the time.
2. Requires the ability to walk 50% of the time.
3. Requires the ability to sit 10% of the time.
4. Physical strength to perform the following lifting tasks:
• Floor to waist - 40 pounds
• 14” to waist - 50 pounds
• Waist to shoulder - 20 pounds
• Shoulder to overhead - 10 pounds
• Carry 40 pounds 30 feet
• Push 40 pounds/force 30 feet
• Pull 40 pounds/force 10 feet
• Carry 20 pounds away from body 30 feet
5. May require climbing, stooping, crouching, handling, above shoulder reaching, grasping, hearing, balancing, kneeling, reaching, speaking, seeing, fine manipulation, depth perception and color vision.
REPORTING RELATIONSHIP
Reports to the Wound Clinic Director.
EDUCATION, KNOWLEDGE AND ABILITIES REQUIRED
1. Registered nurse with current, unrestricted Illinois Registered Nursing license.
2. BLS Certified.
INFECTION EXPOSURE RISK LEVEL
Category I– Maximal Risk - This job usually involves contact with patient or patient specimens. Exposure to blood, body fluids and tissue is possible. The Hepatitis B vaccine shall be made available.
WORKING CONDITIONS
1. Is not required to work under any extreme conditions, but may be required to work around fumes or airborne particles.
2. Must be able to work around a moderate noise level such as in an office environment.
