Position Summary:
The Haven RN Clinical Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Minimum Qualifications
Education: Graduate of accredited school of nursing
Certificate/Licenses: Current, active license as Registered Nurse in Minnesota. CPR certification and valid driver’s license.
Work Experience: Preference given to applicants withexperience providing direct care to residents in assisted living or long-term care setting.
Responsibilities
- Assess and monitor patients’ health status, including vital signs and medical history.
- Administer medications and treatments as prescribed by physicians.
- Assist in emergency medicine situations, responding promptly to patient needs.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Educate patients and their families on health management, treatment options, and lifestyle changes.
- Maintain accurate patient records using electronic health record systems such as Point Click Care.
- Support patients with disabilities by providing tailored care and assistance.
- Manage nursing teams effectively, ensuring high standards of practice are maintained.

- Always demonstrates honesty and integrity in the care and use of resident and facility property.
- Demonstrates respect for co-workers and responds to needs of residents by complying with facility policies on attendance and punctuality and dress code. Able to arrive and to begin work on time and to report for duty on all shifts, weekends and holidays.
