What are the responsibilities and job description for the Case Manager LPN-Case Management-HHK-Sacaton position at Gila River Health Care?
Position Summary : Performs case management of assigned patients to provide timely, appropriate, and compliant care services. Coordinates and verifies incoming patient information regarding hospital admissions to outside area hospitals. Distributes daily log to Case Managers based on patient's primary care provider and tracks and analyzes hospital admissions data to identify trends. Compiles all relevant data into a monthly report for the Case Management Director.Critical Tasks : Organizes confidential patient information to maintain daily log of patient activity.Verifies patients in outside hospitals are admitted as inpatients. Compiles log of patients by primary care provider and shares log with all relevant parties.Tracks and reports on all relevant data and trends regarding hospital admissions, readmissions, and diagnostic groups.Oversees planning and execution of monthly infection prevention education for entire department. Receives infection prevention competency test results and forwards them to the Director of Infection Prevention. Performs administrative duties, such as covering for other administrative positions and taking telephone calls for Case Managers, as needed.Gathers information for insurance authorization for inpatients with insurance.Updates Case Manager and Manager of Hu Hu Kam Memorial Hospital (HHK) Inpatients for the utilization review (UR) process.Assigns patients to Case Manager for logs and follows up accordingly. Performs other job-related activities as requested.Required Qualifications : Graduate of an accredited Licensed Practical Nursing Program required.Current and valid State of Arizona or Compact State Licensed Practical Nurse (LPN) license or ability to obtain the license within 30 days of hire. 1 year of Case Management, home health or hospice, or public health experience required.Current and continuing BLS training and certifications.State of AZ Driver's License with no significant violations during the last 5 years. Demonstrates ability to relate to diverse cultures and specifically the Gila River Community or other Native American cultures and community health services.
