What are the responsibilities and job description for the Social Worker (LSW) position at Gillette Nursing Home?
The Licensed Social Worker is responsible for the overall functioning of the Social Service Department, maintains interaction with family members and responsible parties regarding all aspects of patient care; coordinates discharge planning and in-house transfers, addresses psychosocial needs of patients; acts as contact person for various outside organizations and referring agencies. This position requires the skill necessary to provide continuity in and coordination of patient care (i.e., possesses skills to interview residents, and communicate with community resources, etc.).
Qualifications - LSW
EDUCATION:
Bachelor’s Degree in social work from an accredited college or university.
Professional license as required by the State.
EXPERIENCE:
Experience in working with the people – counselor, patient/family liaison, insurance case services, and government/community organizations.
KNOWLEDGE AND SKILLS:
Leadership and supervisory ability.
Ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures necessary for providing quality services and care.

Pay: $45,000.00 - $55,000.00 per year
