Currently seeking a skilled and driven Social Worker to welcome our ever-growing team!

Day in the Life / Expectations
- Ensure our residents receive the high standard of care they have grown to expect by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences
- Assists with patient evaluation in the Social Services Department in accordance with federal, state and local standards.
- Contributes to ensuring all established policies and procedures are met for related emotional and social needs of the resident.
- Work closely with the healthcare staff members, community agencies and others in an effort to meet the needs of the resident through the development of social service programs.
- Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician

Requirements:
- Bachelor's Degree preferred
- Must be a self-starter able to work independently and efficiently
- Reliable transportation
- Proof of Covid-19 Vaccination
