Part-Time Limited- 10 hours a week
We are currently seeking a Part-Time Wellness Licensed Practical Nurse (LPN) for our day shift (Day Shift (6:45AM-7:15PM); 1 day a week working 12-hour shift). The Wellness Licensed Practical Nurse (LPN) is responsible for working in accordance with established policies and procedures, and/or special instruction from the Assisted Living Manager, Resident Care Coordinator, or Wellness Nurse Manager/Delegating Nurse. Provides nursing assessments, care oversight, and quality assurance for all residents; this includes weekly scheduled appointments, chart reviews, and nursing assessments, as well as nursing intervention and treatments. Maintains a comprehensive Wellness Program, which addresses the needs of both assisted living and special needs residents.
Job Duties & Responsibilities:
- Ensure that the attainment of Ginger Cove Mission Statement takes precedence over all decisions and actions.
- Maintain overall quality of the Resident Care Program in accordance with all local, state and federal regulations and the highest standards of nursing practice.
- Provide clinical supervision of care programs.
- Coordinate with the Wellness Nurse Manager/Delegating Nurse or Director to oversee the quality of resident care.
- Work closely with all staff as a consultant on clinical issues.
- Work with Wellness Nurse Manager/Delegating Nurse and/or Assisted Living Manager to successfully run a comprehensive Wellness Program designed to proactively address physical, psychosocial and rehabilitative needs of the residents. Include quality assurance and outcome measures which monitor trends and anticipate potential problem areas.
- Maintain regular communication with residents, family and physician on individual health care issues.
- In coordination with the Wellness Nurse Manager/Delegating Nurse and/or Assisted Living Manager, maintain a regular schedule of instruction to caregiving staff on clinical and nursing issues.
- Assist with the community programs for infection control, quality assurance, outcomes monitoring and other oversight programs.
- Work with the Wellness Nurse Manager/Delegating Nurse to manage the medication administration program. Perform, supervise or review the check-in process when medications are delivered by the pharmacy.
- Review all resident charts on a regular basis, checking for content, clarity, completeness and appropriateness of care. Responsible for the quality of all resident charts, including compliance with applicable regulations and the highest standards of nursing practice.
- Review monthly physician order sheets and medication administration record updates for accuracy.
- Instruct caregiving staff on clinical and nursing issues.
- Assist the Wellness Nurse Manager/Delegating Nurse or Director in recruiting and selecting caregiving staff.
- Coordinate implementation and follow-up of resident individual service plans with the Wellness Nurse Manager/Delegating Nurse or Director and the caregiving staff. Coordinate reassessments and service plans for residents.
- Coordinate services of outside agencies providing health care or other assistance to residents, including home health agencies, physical therapists, hospice, laboratories, durable medical equipment suppliers and other professional services.
- Review incident reports daily with the Wellness Nurse Manager/Delegating Nurse or Director. Establish intervention and documentation of follow-up.
- Attend classes, seminars and meetings; maintain a high level of knowledge regarding current nursing theory, trends and regulatory changes.
- Perform other duties as assigned.
Qualifications
Job Qualifications:
- High School Diploma/GED required.
- Current certification as a Licensed Practical Nurse in the state of Maryland.
- CPR/First Aid certification or ability to acquire prior to employment.
- Minimum two to three years of nursing experience.
- Knowledge and experience working with the senior community preferred.
- Ability to organize, supervise, delegate and provide leadership.
- Excellent organizational, interpersonal, and communications skills.
- Ability to perform simple arithmetic.
- Computer literacy; ability to document care given.
- Must have completed or be willing to complete educational requirements imposed by State regulations within the specified timeframe.
- Must present self in a professional manner, display genuine concern for elderly and tact in dealing with others.
