What are the responsibilities and job description for the Intake worker and staffer position at GL Home Health Care, Inc.?
POSITION SUMMARY:
LOOKING FOR AN ARMENIAN SPEAKING INTAKE COORDINATOR AND ADMINISTRATIVE ASSISTANT.
Process staffing patients and note taking. These patients must meet the established criteria for Home Health. Basic everyday office operations.
AREAS OF RESPONSIBILITY:
Collaborates with therapists ,managers, therapist companies, physicians and other referral sources for eligibility and appropriateness of home care services.
- Responds to all incoming referrals from eFax and any other referral sources and enters information into the system.
- Manages tracks and processes patient referral paperwork and documents in the EMR system and other systems as appropriate.
- Evaluates referrals for appropriateness to agency services or other community resources.
- Receives and documents verbal and/or written physician orders.
- Sends and Receives Physician orders.
- Office work including faxing orders, filing and scanning paper notes when needed.
- Performs other duties as assigned consistent with skills and training and the goals and mission of the agency.
QUALIFICATIONS:
- Good telephone interviewing assessment skills.
- Self started and fast learner. Able to follow directions.
- Strong organizational skills with ability to work independently
- Possesses strong customer services approach.
Schedule
- Monday to Friday 10am-5pm OR
- Monday to Friday 1pm-5pm to start
Experience:
- Preferred home health experience or knowledge.
