Position Overview:
The Hospice Field Nurse, Clinical Director, and Administrator will serve as the cornerstone of our agency. This individual will be responsible for delivering exceptional patient care, overseeing clinical operations, and managing administrative functions. As a one-person leadership team during the start-up phase, this role requires versatility, leadership, and a commitment to fostering a patient-centered culture.
Key Responsibilities:
Field Nurse Duties:
- Provide direct nursing care to hospice patients, including symptom management, medication administration, and palliative care.
- Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams.
- Educate patients and families on disease progression, hospice philosophy, and care approaches.
- Conduct routine visits, assess patient needs, and respond to emergencies as needed.
- Maintain accurate and timely documentation in compliance with regulatory and agency standards.
Clinical Director Duties:
- Oversee clinical operations, ensuring compliance with state, federal, and accreditation standards.
- Lead and mentor clinical staff as the team grows, fostering a culture of excellence and collaboration.
- Develop and implement policies, procedures, and quality improvement initiatives to ensure high standards of care.
- Serve as the liaison between field staff and administrative operations to coordinate seamless patient care.
Administrator Duties:
- Manage the day-to-day operations of the agency, including budgeting, staffing, and compliance.
- Ensure the agency meets all licensing and regulatory requirements.
- Oversee onboarding and training of new staff members as the team expands.
- Collaborate with referral sources and community partners to build the agency’s reputation and client base.
- Monitor and report key performance indicators, financial metrics, and clinical outcomes to agency stakeholders.
Qualifications:
- Current RN license in OH (required); BSN preferred.
- Minimum of 3 years of experience in hospice care, with at least 1 year in a leadership or administrative role.
- Strong knowledge of hospice regulations, including Medicare Conditions of Participation.
- Excellent clinical skills and a deep understanding of palliative and end-of-life care.
- Demonstrated ability to multitask, prioritize, and lead in a start-up environment.
- Exceptional communication and organizational skills.
- Proficiency in using electronic medical records (EMR) systems.
