What are the responsibilities and job description for the RN Case Manager position at Glades INC?
Glades Talent is working with a standout Home Health Agency looking to fill an RN Case Manager role for the Boca Raton area.
As the RN Case Manager, you will be expected to report in to work Monday through Friday, from 9:00AM to 5:00PM. You will be expected to be rotating ON CALL services with the DON and any future staff.
QUALIFICATIONS:
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- Two (2) years’ nursing experience, preferred.
- Acceptance of philosophy and goals of this Agency. 4. Ability to exercise initiative and independent judgment.
RESPONSIBILITIES:
- Review Dx and establish patient treatment plans
- Monitor visit notes and report changes in patient symptoms or behavior to MD when needed
- Communicate with collaborating physicians or specialists regarding patient care
- Ensure education provided regarding health maintenance and disease prevention is appropriate to patient plan of care
- Assist with Intake as needed and facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members when supervisory assistance is required
- Assures complete and accurate data and documentation are available to make recertification decisions and further assures that those decisions are made on a timely basis
- Acts as a resource and mentor for staff re clinical issues, documentation, inter- and intra- team problem solving and appropriate customer service behavior
- Monitors clinical records and all team activities to ensure that they are accurate, complete and meet required standards in accordance with all regulatory bodies.
- Develops and implements performance improvement activities to respond to service issues and challenges and participates from time to time in program relating to quality and service improvement.
- Ensures that team completes proactive and other patient/family satisfaction surveys, and implements Quality Improvement measures and standards.
- Assists with surveys and requests from regulatory bodies and intermediaries as directed by local program.
- Performs substantive chart reviews to assure there is evidence that quality care is being delivered
- Monitors utilization of resources by every patient to assure cost effective delivery of services
- Orient new staff in the field as needed
- Perform home visits as directed by the DON if necessary
