RN Field Case Manager – Nanuet Area
Glades Talent is partnering with a leading Home Health Agency to find a dedicated and experienced RN Field Case Manager to serve patients in the Nanuet area.
In this role, the Registered Nurse will provide skilled nursing care on an intermittent basis in patients' homes. The RN will work under the direction of the Director of Clinical Services/Clinical Team Manager and in collaboration with the interdisciplinary team to deliver high-quality, patient-centered care in accordance with physician orders and individualized care plans.
Qualifications
- Graduate of an accredited school of professional nursing, with an active RN license in New York State.
- Minimum of two (2) years of nursing experience, preferably in home health or a related field.
- Strong commitment to the mission, vision, and values of the agency.
- Ability to work independently, exercise sound judgment, and demonstrate effective critical-thinking skills.
Responsibilities
- Deliver nursing services as outlined in the patient’s plan of care.
- Conduct initial and ongoing patient assessments, including OASIS data collection, to evaluate nursing needs and determine care priorities.
- Develop, implement, and revise individualized care plans in collaboration with the interdisciplinary team.
- Administer skilled nursing interventions and provide education to patients and families to support recovery and promote health.
- Document clinical notes, progress updates, and care summaries promptly and accurately in alignment with agency policies.
- Coordinate and communicate patient care needs and changes in condition with the healthcare team, including physicians and therapists.
- Act as a resource and educator to patients, families, and caregivers on disease management and care strategies.
- Participate in interdisciplinary team meetings, case conferences, and in-service training sessions as required.
- Facilitate and process physician orders, including certifications, recertifications, and discharge summaries.
- Provide supervision and guidance to LPNs, CNAs, and other team members as needed.
- Participate in quality assurance initiatives, peer reviews, and utilization reviews.
- Ensure compliance with all regulatory and agency standards, including accurate use of ICD-10 coding.
- Participate in the agency’s on-call rotation, including nights, weekends, and holidays.
