Job Summary

Responsibilities
- Deliver comprehensive nursing assessments and develop individualized care plans based on patient needs.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Monitor patient progress and adjust care plans as necessary, documenting all changes accurately.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with healthcare providers in areas such as utilization management, managed care, and behavioral health.
- Conduct diagnostic evaluations and provide care for pediatric patients as needed.
- Maintain accurate records of patient interactions and ensure compliance with healthcare regulations.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in a Level II Trauma Center or similar acute care setting preferred.
- Knowledge of infectious disease care, medical-surgical nursing, and diagnostic evaluation processes.
- Familiarity with utilization management and managed care principles is a plus.
- Strong interpersonal skills with the ability to communicate effectively with diverse populations.
- Ability to work independently while managing multiple priorities in a home health environment.
- Previous experience in pediatrics or behavioral health is advantageous.
