Job Description :
We are looking for an experienced and compassionate Physical Therapist to join our home health team in Brooklyn. The candidate should be committed to performing a minimum of 15 visits per week. This per diem role involves providing individualized care to help patients achieve their therapy goals in a home setting.
Key Responsibilities :
- Evaluate patient physical abilities and limitations.
- Develop and implement personalized therapy programs.
- Work with patients to increase mobility and reduce pain.
- Monitor patient progress and make adjustments to treatment plans as needed.
- Maintain accurate and up-to-date patient documentation.
