Key Responsibilities
- Manage a caseload of home health patients, completing 30-32 points per week.
- Conduct patient assessments and develop individualized care plans using OASIS-E .
- Provide skilled nursing care and ensure high-quality outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Document all care activities in EMR systems such as HomeCare / Home Base or Kinnser Wellsky .
