Glades Talent is working with a leading national home healthcare agency in Garden City, seeking a Full-Time Field Registered Nurse Case Manager to provide :

  • Responsible for overseeing client services.
  • Develops a plan of care for assigned cases, starts cases, and is the point person to families.
  • Communicates regularly with other team members to ensure client satisfaction and safe practices.
  • Supervisory check-in visits throughout the Garden City service area for the geriatric patient populations in their homes.

JOB REQUIREMENTS :

  • Must have a minimum of 3 years' of experience as an RN
  • Must have management / supervisory experience
  • Must have geriatric experience
  • Must be fully COVID vaccinated
  • Must own a vehicle and OK commuting to patients' homes
