Summary
Under the direction of the Behavioral Medicine Unit Manager, the BMU Care Manager/Discharge Planner develops a discharge plan for patients admitted to the Behavioral Medicine Unit. This role assists patients to meet their biopsychosocial needs by providing assessment, mental health counseling and evaluation, mutual goal setting, interdisciplinary treatment planning, agency referral and crisis intervention.
Education, Knowledge, Skills, and Abilities
- BA in Social Work or related field and LSW
- Master's degree in Social Work for an accredited school or university, preferred
- Must be able to work independently as part of a multidisciplinary team
- Crisis Intervention skills
- Therapeutic communication skills
- Ability to interpret medical data as documented
- Strong organizational skills to manage complex activities with tight deadlines
- Effective conflict resolution and negotiation skills
Required Length and Type of Experience
One year of previous mental health experience, preferred.
Required Licensure or Certification
LSW or LISW by state.
Required Physical and Environmental Demands
Requires direct patient contact. Required to walk, stand, bend, and sit. Routine and/or daily tasks or procedures occasionally expose them to blood and other potentially infectious materials. Must be able to remain calm and function clearly in a stressful, potentially violent environment.
