What are the responsibilities and job description for the Admissions Nurse (RN/LPN) position at Glenburnie Rehabilitation and Nursing Center?
Glenburnie Rehabilitation and Nursing Center has an exciting opportunity for a Registered Nurse or a Licensed Practical Nurse to join our team as an ADMISSIONS NURSE to facilitate the admittance of residents to the facility- aiding in the assessment, implementation and evaluation of resident care- and acting as a liaision between Interdisciplinary team members.
12pm-8pm or 11am-7pm

Responsibilities:
- Collaborate with resident, physicians, nursing staff members, therapists and social services department to ensure seamless transfer to the facility
- Communications information effectively to physicians, supervisors, Administrator and any other pertinent personnel in a timely manner
- Ensuring all aspects of admissions are complete including, but not limited to, completing admissions orders/assessments, completion of electronic medical record charting and developing a baseline plan of care.
- Providing a seamless transition with the scheduling of appointments, processing pharmacy orders, etc to maintain continuity of care.
- Assist in admission planning and pre-admission screening.
- Provide support to charge nurses, residents and other team members to exceed resident expectations while providing exceptional customer service.
Qualifications:
- Active and unencumbered LPN or RN license
- Minimum of two (2) yeas experience in long-term care
- Strong computer skills and experience with PCC
- Excellent verbal and written communication skills
- Must be devoted and compassionate about their chosen career
