Position : Licensed Social WorkerDepartment : Acute Care; Med / Surg, Transitional Care, Emergency Services and ObstetricsLocation : Glencoe, MNRemote Work : NoPosition Type : Full TimeHours Per 2-week Pay Period : 64Schedule : M, Tu, Th, F from 8am-4 : 30pmWeekend Rotation : N / AHoliday Rotation : N / ADue to continued growth, we are adding an additional Social Worker role to our organization. This position provides psychosocial assessments, plans care and coordination of care and resources, patient advocacy and crisis intervention assistance to patients and their families. Develops social service action plan / discharge plans using the psychosocial assessment and input from the patient, family, and health care team. The Social Worker works with other interdisciplinary team members to create an environment which supports quality patient care and patient safety.Qualifications :
- Bachelor of Social Work and licensed (LSW) by state of Minnesota.
- Possess knowledge of social work principles and practices. Must demonstrate skills necessary to provide care appropriate to the patient's needs. Ability to make independent decisions when circumstances warrant such action.
- Be a supportive team member, contribute to and be an example of teamwork and team concept.
- Patience, tact, a cheerful disposition, possess enthusiasm, professionalism, as well as willingness to handle difficult patients / interactions. Provides patient care with empathy and understanding.
- Excellent communication skills with the ability to work with a diverse population. Knowledge of and ability to obtain community resources.
- Strong organizational skillet required.
- Prior social work experience or related field preferred.Compensation and Benefits : Compensation decisions are made based off of several factors including relevant work experience, education, certification and licensure as well as internal equity.
