Responsibilities
- Reviews the documentation in the hybrid medical record (electronic and paper medical record) of inpatient and / or ambulatory visits to determine the diagnoses that identify the condition(s) for the patient’s admission.
- Assigns codes for all diseases and conditions documented within the current visit to reflect the Acuity of Illness, POA (present on admission) of our patients. Assigns all codes to reflect Medical Necessity for ambulatory visits.
- Identifies procedures performed for treatment and assigns the appropriate ICD-9-CM and / or CPT-4 codes.
- Groups the visits to the appropriate DRG / APG / APC using the 3M-Encoder and sequencing knowledge, optimizing after assignment of all complications or comorbidities (CC’s) and Major Complications or comorbidities (MCC’s) from the documentation.
- Sends queries to physician “Question Table” for clarification of documentation.
- Educates the physician regarding documentation requirements.
Education:
- High School Diploma
- Medical Terminology
- Anatomy and Physiology via courses or equivalent
- Educational Training in ICD-9-CM and CPT-4 Coding Systems
Experience: Required – Five years Hospital coding, ambulatory or inpatient
Skills/Abilities:
- Computer skills
- Communication skills
- Critical thinking & decision making skills
- Licenses/Certificates/Registrations
- Coder IV– Required - CCS credential

Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $19.32 to $31.89 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
