Team Impact

The Care Manager will be assigned to manage their own patient caseload and may also be required to rotate on-call responsibilities with other Care Managers as needed to ensure 24hour/day and 7day/week care for crisis intervention. The Care Manager will communicate with providers and care team on an as needed basis to discuss changes in patient condition that may necessitate treatment change and will attend regular case review meetings. The Care Manager will work closely with the Interdisciplinary Team to ensure appropriate utilization and provision of resources for patients including referrals to outpatient medical, mental health and substance abuse services. In addition to creating/altering the care plan, the Care Coordinator will ensure that a participant/family values assessment is completed. Caseload will be determined by the level and intensity of care management to be provided.

Responsibilities

• Care coordination for the patients who seek services within the physician practices of Adirondack Medical Services
• Conduct Pre-Visit Planning
• Follow up with patients with new medications to assess response, barriers, and need for additional education
• RN will create/Update Care Plans and work with patients on identifying barriers to treatment goals and assessing self care abilities. Social Worker will follow up with patients regarding any barriers and need for additional education.
• Identify patients who may need referrals to community resources and link patients to community based social support services.

Education/Accredited Program

• Baccalaureate prepared RN or Social Worker or CCM or eligible to sit for CCM or two years acute care/home
• Care and/or case management experience preferred.
• Masters Degree preferred.

Licenses/Certifications/Registrations

• Valid NYS RN license.
• Valid NYS Drivers License required.

Skills/Abilities

• Detail oriented, self-motivated, and a team player
• Ability to work independently as well as part of a team.
• Strong Communication skills are a must.

Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $78,936 to $ 110,839.98 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
