PRN SOCIAL WORKER

Overview :

As a Social Worker at Glenwood Hospital, this pivotal role is designed to support individuals and families through their mental health journeys. This role requires a compassionate, skilled professional committed to delivering high-quality psychotherapeutic services. You will apply a range of techniques and interventions to address emotional and behavioral disorders, tailoring treatment plans to the unique needs of each patient.

Responsibilities :

ESSENTIAL FUNCTIONS :

• Conduct social history assessment of the patient through observation and patient / family interview, obtaining information on the patients needs, skills, interests and limitations for the development and implementation of the treatment plan.
• Coordinate and plan programs and activities to meet patients social and emotional needs.
• Promote recovery, symptom reduction and the achievement of the highest level of social functioning.
• Provides crisis management and family sessions to assist the family in understanding the implications and complexities of a life situation and its impact on lifestyle.
• Assist in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.
• Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.
• Communicate or aid family members in understanding and supporting the patient.
• Manage a caseload and assume responsibility for patient chart documentation.
• Facilitate education, process groups, support groups and referrals as requested.
• Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
